Citizen Request / Complaint Form (PDF)

This document was developed to ensure that citizens have a mechanism to request information or file a complaint with the City of Lake City. All stakeholders are encouraged to use the website https://cityoflakecity.com/contact/, though the attached PDF form can be printed and a handwritten response can be submitted. As always, we will take all feedback, concerns, and/or complaints.  As outlined in the City Charter, the Mayor is the responsible contact, so he or his designee will provide a prompt response upon receipt.  We can take a range of requests/complaints; however, the veracity of complaints will not be fully measured if incomplete or anonymous— we simply don’t have a mechanism to do so…
